Download Certificate of Trade Name and print four copies.  All four copies must then be notarized before you bring them with you in person to our office. Complete the forms with original signatures and bring them with you, along with a $54.00 fee by Money Order, on the day you arrive.

($50.00 registration fee + $4.00 for two certified copies – each additional copy is $2.00)

Hours: Monday- Friday 9:00 a.m. - 5:00 p.m., excluding county holidays*
Fee: $50.00  + $4.00 for two certified copies. Total cost $54.00 (Cash/Money Order only)

Additional certified copies are $2.00 per copy.
